How to Apply for a Child’s Passport
Learn the document and parental requirements for applying for a child’s passport.
🧾 Required Documents
Make an appointment with a Passport Acceptance Agent and bring the following documents with you:

DS-11
-
Passport photo
-
Proof of citizenship

Old passport (if applicable)
-
Copy of each parent’s ID

Swift Letter of Authorization
-
Check or money order
-
International itinerary
👨👩👧 Parental Requirements

At least one parent must attend

Absent parent → submit notarized DS-3053
If a parent is deceased:

Provide death certificate
If a parent cannot be located:

Submit DS-5525
📦 Sealed Envelope Rule
Documents must be sealed by the agent for HAND CARRY.
The agent will charge you a $35 fee to seal the envelope.
Do not open the envelope before shipping to Swift.
