How to Apply for a Lost Passport
Learn how to replace a lost passport and which documents are required.
🧾 Required Documents
Make an appointment with a Passport Acceptance Agent and bring the following documents with you:

DS-11
-
Passport photo
-
DS-64 (generated from DS-11)
-
Proof of citizenship
-
Copy of driver’s license
-
Swift Letter of Authorization
-
Check or money order
-
International itinerary
The agent must seal the documents for HAND CARRY. The agent will charge you a $35 fee to seal the envelope.
Ship the sealed envelope to Swift. Do not open the sealed envelope.
