How to Apply for a New or First-Time Passport
Learn how to apply for a first-time passport and what to bring to your Passport Acceptance Agent appointment.
🧾 Required Documents
Make an appointment with a Passport Acceptance Agent and bring the following documents with you:

DS-11
-
Passport photo
-
Proof of U.S. citizenship

Copy of driver’s license

Swift Letter of Authorization
-
Check or money order for fees
-
International flight itinerary
📦 Sealed Envelope Requirement
Ask the Passport Acceptance Agent to:

Seal the documents in an envelope for HAND CARRY
Ship the sealed envelope to Swift.
Do not open the envelope.
The acceptance agent will charge a $35 fee to seal the envelope.
💡 Important Notes

Do not sign the DS-11 until you are in front of the Passport Acceptance Agent
- Use the DS-11 form link included in the instruction email from Swift
- Passport Acceptance Agent can be found at most US Post Offices and require an appointment. Appointments can be made online at USPS.
