🧾 Required Documents

(Not required for 2–3 week service)
✏️ Renewal with Name Change
Include:

Original name change document (e.g., marriage certificate)

Copy of driver’s license showing new name
🪪 Passport Card Renewal

Renewing book + card → include both

Renewing book only → card not required

Adding a card → check box in both the Swift order form and DS-82 form
💡 Important Notes

Signatures must be original
- Use the DS-82 form link included in the instruction email from Swift
- If the applicant's passport has been expired for 5 years or more, the applicant is not eligible to renew their passport and must proceed through the new passport process

Damaged passports may require DS-11 instead and may not be eligible for the renewal process
