How to Apply for a Passport Renewal

🧾 Required Documents

Send directly to Swift:

• DS-82 

• Passport photo

• Most recent U.S. passport

• Swift Letter of Authorization

• International flight itinerary
  (Not required for 2–3 week service)

✏️ Renewal with Name Change

Include:

• Original name change document (e.g., marriage certificate)

• Copy of driver’s license showing new name

🪪 Passport Card Renewal

• Renewing book + card → include both

• Renewing book only → card not required

• Adding a card → check box in both the Swift order form and DS-82 form

💡 Important Notes

• Signatures must be original

• Use the DS-82 form link included in the instruction email from Swift
• If the applicant's passport has been expired for 5 years or more, the applicant is not eligible to renew their passport and must proceed through the new passport process

• Damaged passports may require DS-11 instead and may not be eligible for the renewal process
