How to Replace a Damaged U.S. Passport
Learn the two procedures for replacing a damaged passport, which form to use, and what additional documentation may be required.

If your passport has been damaged, the replacement process depends on the severity and type of damage.
There are two possible procedures. Your Swift representative will advise which applies to your situation.
🧾 Procedure 1: Renewal With Additional Documentation
In some cases, a damaged passport can be replaced using the renewal process (Form DS-82).

When asked if the passport is damaged, answer No

Include the damaged passport

Include the required damage documentation

You must submit:

A Damaged Passport Statement (provided by your Swift representative)

Your letter must include:

The date
-
“To: U.S. Department of State – Chicago Passport Agency”

“From:” Your full name and date of birth
-
“RE: Damaged Passport”

A brief explanation describing:

How the passport was damaged
-
When the damage occurred
-
End the paragraph with:
“If there are any questions regarding this information, please contact me at xxx-xxx-xxxx.”

Your signature
🧑💼 Procedure 2: In-Person Application (DS-11)
If the damage is more significant, you may need to apply in person.

Complete Form DS-11

Indicate that the passport is damaged

Schedule an appointment with a Passport Acceptance Agent (available at most U.S. Post Offices)

Sign the application in the presence of the agent

Have the agent seal your documents in an envelope and ask to seal for hand carry (the agent will charge a $35 fee to seal the envelope)
The sealed envelope must then be sent to Swift.

Do not open the sealed envelope from the Acceptance Agent

Incorrect form selection may delay processing

The U.S. Department of State determines whether the passport qualifies for renewal or requires in-person processing
Swift cannot override government requirements.
