🧾 Questionnaire Requirements
When completing the visa questionnaire:

You must declare China as a former nationality

You must enter your Taiwanese name in Chinese characters in the section asking for your name in your native language
Information must match your passport records.
📄 Additional Documents to Upload
In addition to standard visa requirements, you must upload:
